Internship Forms
Please return the Learning Agreement to the Career Center on or before first day of your internship. The two Evaluation Forms, one completed by you and one completed by your internship supervisor, should be submitted to the Career Center at the conclusion of the internship. The Career Center will provide copies of all internship forms to your academic advisor.
