Support for Accommodation Request (SAR)
The SAR is intended to assist students with the transition from high school to their chosen post-secondary school of choice by placing the relevant and vital student information regarding accommodations into one document. The SAR should be completed by a group of people who know the student best and who have access to the needed sources of information (IEP’s, assessments, 504 plans, evaluations, high school records and etc.). People in this group would be the student and any of the following who would be helpful in supplying the necessary information; parents, special education teacher, regular education teachers, transition coordinators, Vocational Rehabilitation personnel, administrators and relevant Area Education Agency personnel.
The link below is a map of the local Iowa School Districts who have teachers who have completed training in the SAR documentation process. Each school district will also have an Area Education Agency representative who has also completed the SAR training.
