Documentation of a Disability

A well written Individualized Education Plan (IEP) with a Summary of Post-Secondary Living, Learning, and Working form or a 504 Plan may include the appropriate documentation for Waldorf College to provide you with your accommodations.

If your documentation is not complete you will be asked to obtain additional evaluative information before we may legally provide the requested accommodations.

Waldorf College in participation with the Iowa Department of Education, Iowa Area Education Agencies, local high schools and other post secondary educational institutions around the state are piloting a documentation process, Support for Accommodation Request (SAR), to ensure that appropriate accommodations for students transitioning from high school to a college setting are sufficiently documented so that student’s accommodation requests are able to be processed more efficiently.
