Student Medical Insurance Information

Waldorf College is vitally interested in making your academic and personal experience meaningful and successful. Your health and wellness are key components of that experience.

Primary Coverage

Waldorf College requires all full time students (12 or more credit hours) to carry health insurance, either through a family employee health plan, a private insurer, or through the plan offered by the college through Bollinger Insurance. As a full time student, you have been enrolled in the Bollinger Plan and the premium charge has been posted to your student account. It is payable with your tuition bill unless you waive coverage before the September 10, 2012 deadline.

Athletes will not be allowed to participate in practices, conditioning, or games until proof of primary insurance is presented or with Waldorf's offered plan through Bollinger Insurance.

Plan Highlights for 2012-2013 include:

  • Coverage 08/01/12 - 07/31/13
  • Covers illness and injuries
  • Covered benefits include, but are not limited to outpatient & inpatient physicians' fees, hospital charges, x-rays, lab work, ambulance and prescriptions (with limited maximum benefits)
  • Preventative services are NOT covered under this plan
  • Co-payment per cause is $25
  • Annual cost:
    • Domestic Student: Unknown at this time, last year’s rate: $628
    • International Student: Unknown at this time, last year’s rate: $788

Students will be enrolled automatically in the Waldorf plan and only need to take action if you wish to waive out of the plan. If you are looking at waiving out please consider the following:

  • HMO or PPO networks may not provide adequate coverage for providers in the Forest City area. Most HMOs and Medicaid, other than Iowa Medicaid, is not adequate coverage to waive
  • It is 40 minutes to the nearest Emergency Room and Urgent Care facilities and not all accidents and illnesses requires an Emergency Room visit or Urgent Care, which sometimes is the only coverage a student has with out of network insurance. Primary insurance that only covers Emergency Room visits and/or Urgent Care visits is not adequate coverage to waive
  • Students who may be losing their health insurance coverage due to graduation, family coverage changes, or other circumstances may want to consider not waiving the offered Bollinger Insurance plan

How to waive the Bollinger Insurance Plan:

  • After July 1, 2012, visit Bollinger's web site at www.BollingerColleges.com/Waldorfarrow-out.  Compete the requested information OR
  • Send a copy of both sides of your health insurance care with a written request to waive the Bollinger Plan to Beverly Retland, Student Insurance Coordinator, 106 South 6th Street, Forest City, IA 50436 or email scanned copies to retlandb@waldorf.edu or fax to 641.585.8194 by September 1, 2012.

The deadline to waive coverage is September 10, 2012. Sending insurance information to the Student Health Services and/or Athletic Trainer DOES NOT waive a student off the Bollinger Plan. ONLY the two options above will waive the Markel Plan and premium!!!

Secondary Accident Insurance

Waldorf College carries secondary accident insurance on all full time students only. This insurance will piggy-back on to the student's primary health insurance and pays deductibles, co-insurances, and co-payments. More information may be found by calling Beverly Retland at 641.585.8144.

Waldorf College never pays for any provider visits. Bills are always the student's responsibility.

Waldorf College Athletic Supplemental Accident Insurance

Waldorf College student-athletes are automatically enrolled in a Supplemental Accident Insurance Plan from August 1 through July 31 each year. Please note, this is for accidents and injuries only. It is also an excess plan and never pays as primary; it does not cover illnesses, and only pays after the student's private health insurance has been processed.

It is always the student's personal responsibility to pay their medical bills. All student-athletes need to rely on their own personal health insurance at all times. Please note the following important considerations with respect to this insurance:

  • That the accidental supplemental insurance is filed only after primary insurance has been processed and only under certain conditions.
  • The injured student must see a licensed provider within 30 days of the incident or this insurance may reject the claim.
  • That claims arising from self-inflicted injuries will not be covered.
  • That claims arising while the student is under the influence of drugs or alcohol will not be covered.

Any amount not covered by the student's personal health insurance or the supplemental plan is the responsibility of the student athlete and not the responsibility of Waldorf College. Waldorf never pays for any provider visits. Bills are always the student's responsibility.

It is important to see or contact the athletic training department or health services immediately following any injury, unless it is an emergency. If the situation is an emergency, the student-athlete must still notify the athletic training department or health services within 48 hours of the emergency room visit.

The athletic training staff or health service staff, along with the student-athlete, will determine the correct course of action following an injury (ie. ER, Family Physician, Orthopedist, treat in house etc...) If a referral to an outside source is necessary, the athletic trainer or health services will follow the necessary procedure in filling out a claim form. Failure to do so may result in any cost pertaining to the injury becoming the responsibility of the student-athlete and/or his/her parents(s), guardians(s).

An injury report must be made to the coach or athletic trainer immediately when the accident/injury occurs. This is how the secondary insurance will verify the claim. The athletic trainer or your coach will be able to give you your secondary insurance card to present to your provider.

When seeing a provider, present the student’s personal health insurance card and Waldorf's secondary insurance card at check in. The provider will submit your claim to your primary insurance and then to Waldorf's secondary insurance. Any balance not paid by either insurance company is the student's responsibility.
